The Applicant hereby applies with Bank of Kaohsiung (hereinafter, "the Bank") for the "Online Counter- Advance Data Entry" Service and shall duly observe the terms and conditions agreed as follows:

Important notice to Advance Data Entry Service【20200907】

1. The "Online Counter – Advance Data Entry" service (hereinafter, "this Service") refers to the request of the Applicants for different banking services with the Bank at any of the banking units (the scope of available services will be posted by the Bank) thereby filling in the application in electronic format at the Online Counter placed at the official website of the Bank. Through this process, respective banking units of the Bank can provide related services more efficiently.
2. The Applicant can just fill in the application in electronic format in advance under this service, and still have to visit any of the banking units of the Bank to proceed with subsequent procedure in order to complete full service. The Bank reserve the right to reject or accept the content of application submitted by the Applicant.
   
3. The Applicant agrees to comply with relevant requirements of the Bank in providing relevant services as stated in the previously filled application form in electronic format, and shall bring related identification documents, passbook, the specimen seal on impression card, and other necessary documents to respective banking units of the Bank to proceed with subsequent procedures, and comply with the rules and regulations of the Bank in the request for identity authentication.
   
4. The Applicant shall visit a banking unit of the Bank to continue the transaction procedure within the effective period of 7 days (in calendar days from the day after the application, including recognized holidays and weekend holidays) after completing the online application form. The Bank will delete the application information beyond the aforementioned deadline.
   
5. In case of error found in the information provided by the Applicant in the application, or the information provided is found irrelevant with the requirement of the branch in related transaction services, to the effect that the Bank cannot process the subsequent procedure of transactions for the Applicant, the Applicant shall fill in an application or related forms again in hard copy for processing the subsequent procedure of transaction.
   
6. As a matter of principle, the banking units of the Bank will provide priority service for those who have filled in the application form in electronic format in advance. However, priority service is subject to change depending on the actual status of operation at respective banking units, which means that the Applicant may not be certainly entitled to priority service.
   
7. If the Applicant clicks to agree, it shall be deemed the full understanding and agreement of the Applicant of the terms and conditions underlying this Service.
